Job Description
About the role
The Service Coordinator is responsible for developing the existing group programs for adults in hub and in other community settings and individualised support. The incumbent provides client centred leadership to a team and facilitates teamwork to enable the delivery of safe and effective, individualised, person centred supports for people living with disability to assist them to achieve their goals. The position contributes to Novita services more broadly by collaborating with other Novita staff and the community to support people where they want and facilitate staff movement across the range of support types to meet client needs.
This Position is paid in accordance with the SCHADS Award, level 4.
As a Service Coordinator, you will:
identify the needs of clients and respond by linking them to existing services and developing new services
develop a program of activities which is engaging, offers opportunities for skill development, developing independence and forming relationships through community participation
lead a team of Senior Support Workers and Support Workers, allocating and organising staff supports for clients in line with their funded model of care and support requirements
provide supervision and coaching to the Senior Support Workers and Support Workers in the team to enhance their practice and the outcomes for clients
actively identify and manage risks and incidents when they occur to support the safety of both clients and staff
deliver quality service improvement in line with the ‘Stepping into the Future’ project and SCERTS
drive operational performance to deliver service, financial and business goals.
